TEAMS Student Instructions

The  form  is designed to collect all entries and deeper information based on feedback from the program audit to ensure continuous improvement. The directions guide the proper way to complete.  The form allows the convenience of completion from a mobile phone, computer, or tablet. The form is available to  students  outside a Tennessee State University (TSU) email. 

Section 1   All areas of the form must be completed.

This section collects the date, observer, and school name.  

• Date - The date is embedded, so you only need to click and choose the date. 

• Observer Name – This is a drop-down menu to choose your name.  

• School Name – This is a drop-down menu to choose the school. 

Section 2  

This section collects the student demographics and program characteristics.  

• Student Name – The input requirement is Last, First. This must be consistent for data purposes. 

• Race –is a drop-down menu to choose the student’s race.  

• Gender –is a drop-down menu to choose the student's gender. 

• Major - is a drop-down menu to choose the student's major. 

• Program Type - is a drop-down menu to choose the student's program type. 

• Clinical Experience - is a drop-down menu to choose the student's clinical experience. 

• Visit Type – a choice between whether announced or unannounced.  

• Observation Number - is a drop-down menu to choose the number of visits. 

Section 3  

This section includes TEAM areas for observation in a Likert format. Concluding the scores are the areas to address reinforcement, refinement, and reflections.
