BSSW Field Education ManualNote the Forms that Facilitate your Progression.

Please review all documents below. The following forms are designed for users to download to their PC, save, then complete, and forward and/or print as needed. (Do not complete in a web browser and try to save and/or print). In order to successfully upload the following forms please click save to my documents, close out of the website, and pull up the document in your documents; then work out of your documents. Some files were created in pdf, you will not be able to make changes to pdf forms.

Prospective Students
Review the Prospective Student Introductory Packet. Meet with Advisor to complete your(New) Bachelor of Science In Social Work Suggested Four Year Plan Scholarships: Please note: Students that are interested in applying for our scholarships must apply through the TSU Foundation office is located on 1st Floor of the Goodwill Manor Building. *Freshman Year*
Complete the Student Cover Sheet Verify receipt of the BSSW Student Handbook. Meet with Advisor to complete your Bachelor of Science in Social Work Suggested Four Year Plan Attend the BSSW Student Orientation Register as a member of the Social Work Club. Review and become familiar with the TSU Portfolio Review and Evaluation Process (PREP)
Sophomore Year
Review the guidelines in the Student Handbook. Meet with your Advisor to discuss your progress. Complete the Formal Application to the Social Work Program. Schedule a Formal Application Interview with your Advisor.
Junior Year
Receive your formal Letter of Acceptance into the Social Work Program. Apply for Alpha Delta Mu Honor Society Register for your 1st SOWK Elective. Attend the Field Practicum Informational Meeting. Complete Field Practicum Application and Senior Standing Checklist in a "Readiness for Field" advisement with your Advisor.
Senior Year
Register for your 2nd SOWK Elective. Review and become familiar with the BSSW Field Education Manual Field Education Placement Agency List Undergraduate Field Instruction Application Baccalaureate Field Education Learning Contract Evaluation Field Education Activity Sheet Field Education Agency Information Form Practice Behavior Abilities Evaluation Form Student Evaluation of Internship BSW Field Education Timesheet Termination of Field Education Placement Register as a member of and obtain Liability Insurance from the TN Chapter of the National Association of Social Workers (NASW). Review and verify Senior Standing Checklist for inclusion in your Portfolio. Review and complete the requirements listed in the TSU Portfolio Review and Evaluation Process (PREP) Schedule and attend a Portfolio Review meeting with your Advisor. Schedule and complete the Senior Exit Examination. Complete the ACAT examination on the date scheduled.
