All PARFs are now located in Dynamic Forms. To submit a PARF, click on the corresponding form link below.
To correct previously submitted personnel actions (PARFs), please note, in the 'Special Conditions' section of the form, the reason for the correction.

EDUCATIONAL ASSISTANCE FORMS
FAQ: Processing of Educational Assistance Documents TBR Guidelines for Educational Assistance (view PDF to right) Please Note: If employee plans to enroll in more than one class per semester (i.e., one free course and additional course(s) requiring reimbursement), please follow the below steps: Complete the Request for Education Assistance form for the free course: Complete the Request for Tuition Reimbursement Form or course(s) requestion reimbursement (ensure completion before enrollment). For the Tuition Reimbursement Form, obtain signatures from designated person(s) and submit to the HR department within 14 calendar days of course semester start date> (Any late submission applicant must provide justification of late submittal). Request for Tuition Reimbursement Form (max 6 credit hours)
Note: Applicant must be employed 6 months prior to term and class first day. Reimbursement Process for Payment - At the end of the current semester, Submit a Copy of final semester detailed payment Receipt/Statement, and a Copy of current semester Final Grade(s).

State Employee Fee Waiver (formerly PC 191) and/or Audit/Non-Credit Program EMPLOYEES: In order to take a course at the University, follow these steps:

REGISTER
Then, complete the above Fee Waiver Form, have your manager approve it, and send it to Jo Mercer in Human Resources for approval. If you are taking the class at TSU, HR will forward the approved form to the TSU Bursar’s Office. If you are taking a class at another institution, you can either pick up the approved form from Human Resources. |
